How to refer a pet
Here are the different solutions:
- Call the clinic directly. A direct veterinary line is available to referring veterinarians. This number must not be passed on to your pet owners.
- Fill in a paper liaison form and give it to the owner of the animal you are entrusting us with. It can be downloaded from the Agoravet.fr website. A block of printed sheets (scheduler type) can also be sent to you on request.
- Fill in the online form. You can attach various documents, such as your medical history or the results of any tests you have already undergone. We will receive your request directly.
- Send us your request by e-mail to [email protected]. In this case, please enclose any additional examination results, even if normal (e.g. X-rays, blood test results, histological analysis results, etc.).
An internal medicine and ultrasound service is available. This service is provided by Dr Olivier GUILLAUME.
Terms and conditions :
WITHOUT A DIRECT REQUEST FROM YOU, NO PATIENT WILL BE TAKEN ON.
A liaison document specifying the nature of the request, the complete medical history and the main results of examinations already carried out is also required.
You will receive a notification when the animal and its owner are received at the clinic.
A care report is sent to you when the treatment has been completed and the results of the various examinations in progress have been received.
If, at your request, medical or surgical follow-up is provided by the Agoravet center, a notification is sent to you after each follow-up visit to keep you informed of the progress of your patient's care (telephone call, e-mail or written report if necessary).
In the event of any difficulties in transporting patients, an animal ambulance service can be called upon to speed up urgent care.
